This notice explains the information Financial Foundations asks for in its public request form, why it is collected, and how to use the site without sharing high-risk information too early.

Information you choose to provide

When you send a request, the form asks for your name, email address, phone number, state connected to the search, optional former name and last known city, language preference, selected search path, and your contact consent. The form does not ask for a Social Security number, account number, copy of identification, or banking credential.

Do not submit Social Security numbers, bank credentials, account numbers, copies of identification, payment card details, or other sensitive documents through the public request form.

How Financial Foundations uses the request

Financial Foundations uses the information you submit to respond to your request, help organize an official-search starting point, and, if you request it, discuss whether optional administrative assistance may be appropriate. A form submission is not a state claim and does not determine eligibility, ownership, timing, or recovery.

The public request form is designed for an education-first first step. You may always use the official state process directly instead of requesting optional assistance.

Payment information

Payment card information is not collected through the public request form. If you later choose an eligible optional administrative-fee path, checkout opens on a Stripe-hosted payment page. Financial Foundations does not receive or store your complete payment card number through this website.

Stripe processes payment information under its own terms and privacy practices. Review Stripe’s current materials before completing an optional payment.

External programs and sources

State unclaimed-property programs, government websites, and third-party resources operate independently from Financial Foundations. Their websites, forms, claim requirements, and privacy practices are their own. Verify the destination before providing any document or personal information.

Questions or changes

Use the contact page to begin a non-sensitive support request. Please do not use any public web form to send high-risk identity, financial, or payment information. This notice may be updated as the site or its request process changes, and the effective date will be revised when that happens.
